What is Hospice Care?

Hospice care is considered appropriate for terminally ill patients with a life expectancy less than 6 months.
During this time, the hospice team will focus on the patient’s comfort, including their emotional and spiritual well-being, rather than curing their illness. For that reason, the patient may be prescribed medications and therapies to alleviate pain, shortness of breath, and other symptoms that would otherwise reduce the quality of life and prevent interaction with friends and family during their final days.

Hospice care is also intended to help the family and can provide crucial emotional and spiritual support before and after a loved one’s death.

Types of Hospice Care

Terminally ill patients may receive one of four levels of hospice care, as defined by the Medicare Hospice Benefit:

  • Routine Home Care: Provided in the patient’s residence, including private homes, assisted living facilities, and nursing homes.
  • General Inpatient Care: Becomes available when efforts to manage symptoms in any other setting have proven ineffective. General Inpatient Care can be provided in a hospital, hospice inpatient facility, or nursing facility that offers 24-hour skilled care.
  • Continuous Home Care: This type of hospice care is provided between 8 and 24 hours a day to manage pain and other acute medical symptoms. Continuous home care must be predominately nursing care but may be supplemented by a caregiver and hospice aide services.
  • Inpatient Respite Care: Offers temporary relief to a terminally ill patient’s primary in-home caregiver. Respite care is provided for up to 5 days in a hospital, hospice facility, or long-term care facility, so long as the 24-hour nursing staff is available on all shifts to ensure the patient’s needs are met.

Options for Hospice Care

According to the National Hospice and Palliative Care Organization, more than half of those who die in hospice receive care at home, while roughly 1/3 receive care at a nursing home, assisted living residence, or other long-term care facilities.

Most stand-alone hospice facilities offer in-home care. In such a case, a family member will serve as the patient’s primary caregiver, aided by a team of nurses, aides, a social worker, and, if the patient desires, a clergy member or spiritual advisor, who’ll make regular visits to the home. The hospice service also provides needed medical supplies, including hospital beds, wheelchairs, and incontinence supplies, usually delivered to the home, as well as any prescriptions related to pain and comfort management.

Because in-home hospice care isn’t governed by visiting hours, family and friends can come and go as they please.

Long-term care facilities, including nursing homes and assisted living, might have a hospice care team on staff, while others will contract an outside service to provide hospice care to terminally ill patients. In either case, it’s the responsibility of the nursing home or assisted living facility to coordinate with the hospice provider, handle the patient’s routine medical needs, and furnish any medications and supplies not related to the patient’s terminal illness.

Depending on the facility’s policies, family and friends may be restricted to regular visiting hours. Members of the hospice team and facility staff will be caring for multiple residents, and may not have the time or inclination to interact with or provide emotional support to a patient’s family. Finally, unless the long-term care facility contracts with multiple hospice services, the patient and their family won’t have an opportunity to choose one that best meets their needs.
