
Reverse Osteoarthritis Damage With One Injection?
Mention the word “osteoarthritis,” and you might immediately think of people you know who have it. In fact, you might have this degenerative joint disease yourself and know just how much pain it can cause.
There is no cure for arthritis, and the structural damage to cartilage cannot be reversed. Fortunately, a University of Colorado-led research team has raised the hopes of osteoarthritis sufferers with a promising new treatment.
Scientists and engineers from the University of Colorado Boulder (CU Boulder), CU Anschutz, and Colorado State University developed an innovative treatment that helps aging or damaged joints repair themselves in just a few weeks.
The new therapies involve a single injection into a joint and a kit of biomaterials and proteins that recruit the body’s own cells to repair damaged cartilage. The team members used this new approach in animal studies and expect to continue their research with humans.
“Our goal is not just to treat pain and halt progression, but to end this disease,” Stephanie Bryant, a professor of chemical and biological engineering at CU Boulder, who led the research team, told CU Boulder Today.
What is Osteoarthritis?
Osteoarthritis is the most common form of arthritis and affects about 33 million adults in the United States, and as many as 240 million people worldwide. While many older adults suffer from Osteoarthritis, younger people are affected by it as well. The degenerative joint condition affects one in six people over the age of 30.
Osteoarthritis is also known as a “wear and tear” disease because the protective cartilage at the ends of your bones that connect the joints wears away over time. This results in “bone-on-bone,” as it’s called, when bones rub together, and can cause pain, stiffness, inflammation, and mobility problems. Over time, it can also damage the bone, deform the joint, and cause severe pain during movement. Osteoarthritis primarily affects the hands, knees, and hips.
How The Research Team’s New Therapies Work
Since there is no cure for Osteoarthritis, patients are limited to either treating the pain or replacing the joint. Now, the Colorado research team offers two new options.
The first strategy involves a single injection in an infected joint that releases a drug in small doses for months. The drug, already approved by the Food and Drug Administration, was repurposed for Osteoarthritis.
The second approach is intended for more advanced cases, such as those who have significant lesions in cartilage or bone. The team developed a cocktail of engineered proteins that can be injected through minimally invasive procedures. Once inside the body, it attracts the body’s own progenitor cells to fill and regenerate damaged areas of cartilage or bone.
When the investigators used the injection to treat animals with arthritic joints and injuries, the joints returned to a healthy state within four to eight weeks. In more severe injuries, Bryant said the team saw “full regeneration and repair of the defect.”
“In two years, we were able to go from a moonshot idea to developing these therapies to demonstrating that they reverse osteoarthritis in animals,” Bryant told CU Boulder Today.
When the team experimented with human cells from patients who had joint replacements, the therapies showed a real ability to help cells regenerate.
CU Research Team Receives Grant
Because people struggling with Osteoarthritis have few alternatives to ease their pain, the U.S. Department of Health and Human Services (HHS) has allocated millions of dollars to various initiatives investigating a cure for Osteoarthritis.
To help people with Osteoarthritis have more treatment options, the Advanced Research Projects Agency for Health (ARPA-H), an agency within the HHS, is investing heavily in projects aimed at finding a cure for this disease.
Evalina Burger, a professor and chair of the Department of Orthopedics at CU Anschutz, has noticed osteoarthritis affecting people of all ages. Burger said she has seen it in grandparents who struggle to comb their hair without shoulder pain, as well as runners and hockey players who stopped playing because of knee or back pain.
“At the moment, the options for many patients are either a massive, expensive surgery or nothing,” Burger told CU Boulder Today. “That’s why ARPA-H is so important.”
The ARPA-H program that aims to eradicate Osteoarthritis by enabling joints to heal themselves is called Novel Innovations for Tissue Regeneration in Osteoarthritis (NITRO). The Colorado investigators received a $33.5 million grant from NITRO to begin the next stage of their research.
“It’s super exciting to be a part of the very first program of ARPA-H and to be one of the first teams to advance to the second phase,” Bryant told CU Boulder Today.
If everything goes according to plan, the research team hopes to begin human clinical trials within 18 months. In the meantime, the investigators want to publish their animal findings in a peer-reviewed journal later this year. They have also formed an independent start-up company called “Renovare Therapeutics Inc, to begin commercializing their innovative therapies for Osteoarthritis.
“This could be a real game-changer for patients,” Bryant told CU Boulder Today.
